1. Fill out forms before your appointment: Use the links below to access and fill out forms and bring those forms to your appointment.

2. Bring ID, health insurance cards, and forms: Make sure to bring a valid government issued photo ID, your health insurance card, and any pre-filled out forms to your appointment. 

   • If the patient is less than 18 years old, the parent/guardian must present their photo ID and must be present at the appointment.​

3. Previous medical records: If you have them bring the patient's vaccination records and/or any previous medical records. Otherwise, come prepared with your previous healthcare provider's contact information (i.e. phone number and fax number). You can also fill out the medical request form below and give the form to your previous healthcare provider so they can send medical records to our office.
